Please call our offices to make an appointment. Our Springfield Office number is (877)547-9101 or (217)547-9100. Please refer to the “Locations” section of this website for the phone numbers of our other offices. Appointments can also be requested online, please refer to the “Request an Appointment” section of this website for the appointment request form.
Our Springfield Office is located at the corner of Old Jacksonville Road and Koke Mill Road, which is a quarter of a mile west of Veterans Parkway (Route 4) on Old Jacksonville Road. The building sits in the southeast corner of that intersection. Please view our Locations section for detailed maps.
You will need the patient’s Social Security Number as well as payment or proper payment arrangements at time of service. For more details, please refer to our self pay, worker’s compensation, or liability sections in our financial policy.
As a courtesy, we do submit insurance claims for our patients. It is important that we have your correct insurance information. We also accept cash, checks, debit cards, and VISA or Master Card. We can accept credit card payments by phone.
Our patient accounts staff will be happy to discuss your account and arrange a payment plan with you upon your arrival at our offices.
